Home Visit Lactation Consult - Followup

This offer is for existing clients of Dream Baby Cafe who have received a Lactation Consultation only and are following up for the same problem

In this visit you will receive:

Review of previous problem and plan
Review of progress toward plan
Assessment of feeding
New identification of goals
Customized Written Plan
1 week of followup
Written Summary sent to Health care provider
